II Proposal
A. Describe your proposal and why it needs to be implemented. (Example: The purchase of CPR equipment is proposed. The students need to learn CPR).
B. What students will benefit from this proposal?
C. Is this an upgrade to an existing program or startup of a new program? (Example: This new program is an extension of the health curriculum)
D. How does the proposal support the curriculum objectives of our schools? Are other schools currently using this program? (Example: First aid is a topic in the 7th grade health curriculum).

III Proposal Objectives
A. What is a measurable outcome for your project? (Example: Twenty students will learn CPR each marking period).

IV Methods
A. Describe the activities to be conducted to achieve the desired objectives. (Example: CPR training will be incorporated into the 7th grade health class. Certified personnel from the Red Cross will conduct the training every Thursday for one marking period).
B. Which teachers and/or volunteers will be responsible for the implementation of the proposal?
C. What alternative methods were considered, if any? (Example: The students could attend CPR class at the YMCA for an entire day.)

V Follow Up
A. Explain how and when you will evaluate the success of your program in cooperation with a Foundation Trustee? (Example: Upon completion of the training the Trustee will be notified.)

VI Budget
A. Will the amount of money requested (section I D) entirely fund the proposal? If additional funds are needed, how will they be obtained? (Example: This grant will cover the cost of the CPR equipment but funds will be requested from the Board of Ed to pay the trainer.)
B. Is this a program that will require additional funding at a later date? (Example: Would a CPR trainer have to be paid every year?)
